The Center for Medicare Advocacy (CMA) has announced their next webinar, slated for December 11, 2013 from 2-3 est. The topic is Discharge Planning & Care Coordination: Tips for Advocates. The announcement description from the press release explains:
On May 17, 2013, the Centers for Medicare & Medicaid Services (CMS) provided additional guidance for enhancing the discharge planning available to Medicare beneficiaries, clarifying the roles of hand-off and receiving care settings, and provided suggestions for making care transitions, and care coordination across care settings more robust. In addition, the Affordable Care Act (ACA) introduced a number of quality of care and payment measures that give additional vitality to Medicare discharge planning as an activity and resource for improving health care outcomes for Medicare beneficiaries... [L]earn about key features of Medicare’s expanded guidance on discharge planning and highlight provisions of the ACA that can be used as tools to help beneficiaries obtain the discharge planning and care coordination services they need.
